The following forms and worksheets should be downloaded to assist you with your application process. You should make copies of the Clinical Observation Letter and Forms as needed; please use a separate observation form for each site.
Full application packets to apply to the PTA Program will be available on October 1, 2021. There are additional forms in the application packet that are not available online. All forms in the application packet must be submitted by February 4, 2022 for consideration for a seat in the PTA Program beginning in August 2022.
